MEMBERSHIP TERM
New Memberships have a one year minimum commitment for both Monthly and Yearly payment plans. After the first year, Membership continues with 30 day notice required to terminate.

LAB PRICING
We are able to offer lab pricing at the low wholesale rate we receive, which we pass along to members to help you save money.
PRESCRIPTION PRICING
We are also able to offer low prescription drug pricing by also passing along the wholesale prices we receive directly to members.

Additional charges may apply for vaccinations, splinting supplies, certain bandages, and other necessary tests or supplies as deemed necessary between the physician and patient. All fees will be discussed prior to administration of treatment with full consent of the patient or an appropriate representative.
